Return and Refund Policy for Paperwork Services

At Clearmind Clinic, we understand the importance of providing clear and effective documentation for our patients. While our clinic offers paperwork-related services, including psychiatric evaluations, forms, and treatment plans, we do not engage in the sale of physical goods or items for resale. As such, our return policy applies solely to services rendered for paperwork that is charged out of pocket.

1. No Return or Refund for Completed Paperwork
Due to the nature of the services we provide, once paperwork is completed, processed, and delivered (in physical or digital form), we are unable to offer a return or refund. This includes, but is not limited to, psychiatric evaluations, treatment plans, insurance forms, and any other related documents.

2. Refunds for Payment Errors or Unfulfilled Services
If there was an error in billing or the requested paperwork was not provided as agreed upon, please contact our office immediately. We will review the issue and, if applicable, issue a refund for any overpayment or unfulfilled service.

3. Cancellations and Adjustments
If you wish to cancel or modify a request for paperwork services before it has been completed or processed, please contact us as soon as possible. Cancellations may be subject to a processing fee depending on the stage of completion.
